KeymasterWe understand your requirement, but we have create footer section using widget in this theme, that’s why you may please use footer widget for that or you can customize or add new section if you know coding knowledge.

Thank YouKeymasterWe are using “Contact Form 7” plugin and shortcode for that, you can use same plugin or you may use any contact form plugin for that, just put that plugin shortcode using following steps
Go to Dashboard >> Appearance >> Customize >> Home section >> Appointment SectionYou can show “Form Shortcode” option here, just put your form shortcode.
