For adding contact form shortcode in Appointment Section, i’ve followed your steps but still inactive and in the contact us page there’s no Add contact form shortcode.

For Services, I’ve created six sections with different Icons, only the last section not appeared.

What do mean with WordPress credentials? do you need Username and Password?
